An F1 students visa status during OPT is now dependent upon the timely reporting of students change of address and employer information. Failure to report this information will cause the student to be out of status and could result in the termination of their SEVIS record.

IMPORTANT! PLEASE READ: Your Student and Employer Update form will be REJECTED and you will be asked to resubmit the form if you do not use proper spelling and capitalization. DO NOT write in all lower case letters (john doe) or in all upper case letters (JOHN DOE). Capitalize the first letters of each word (John Doe). Make sure all information submitted is the correct spelling.
